The NOFO emphasizes that arthritis impacts are not evenly distributed: prevalence and activity limitations are higher among people who are older, have higher body mass index, are physically inactive, have poorer physical and mental health, and face greater social disadvantage.
A central problem the NOFO aims to solve is underuse of strategies that are already known to help. About one in three adults with arthritis is inactive, and only around 11 percent have participated in an educational self-management program. While many primary care providers do recommend physical activity (about 58 percent of the time), there is substantial room to improve both the frequency and quality of counseling, including routine physical activity assessment. The NOFO also highlights a major gap in referrals to arthritis-appropriate evidence-based interventions (AAEBIs): approximately 65.5 percent of primary care providers do not recommend AAEBIs, most commonly because they are not aware these specific interventions exist or how to connect patients to them. On the patient side, fears that physical activity will worsen pain or accelerate disease progression can be a powerful barrier, but these concerns can often be addressed through education, reassurance, and structured programs that make activity safer and more approachable.
The NOFOs overall purpose is to support dissemination of AAEBIs and the creation or strengthening of referral pathways so more adults with arthritis can access and participate in these interventions. The CDC is explicitly seeking approaches that increase the proportion of adults with arthritis who receive physical activity counseling from health professionals, increase participation in AAEBIs, reduce health disparities, and improve health outcomes. A key theme is sustainability: funded activities are expected to be carried out in collaboration with stakeholders and aligned with national and state initiatives and other CDC-funded programs, including building cross-referral relationships between interventions and partners so systems keep working after the grant period ends.
Health equity is a major focus. The NOFO frames limited AAEBI availability and accessibility as a driver of disparities, because people in high-burden or underserved communities may have fewer local programs and fewer provider referral options. It prioritizes making AAEBIs and related counseling accessible to populations disproportionately affected by arthritis, including (but not limited to) veterans; uninsured or underinsured adults and Medicaid beneficiaries; adults living in rural, frontier, or other medically underserved areas; people with arthritis-attributable work or activity limitations; those with severe joint pain, disability, or moderate to serious psychological distress; and racial and ethnic populations experiencing higher prevalence or burden, including American Indian and Alaska Native, African-American, and Hispanic/Latino communities. The intent is not only to increase overall participation, but to close gaps in who benefits from effective arthritis management support.
Funding is organized into two components that reflect different stages of readiness and scale. Component A is centered on capacity and infrastructure building. It supports groundwork needed to disseminate AAEBIs and to raise awareness among health care providers about physical activity counseling, physical activity assessment, and AAEBIs, including how and where patients can access them. Component B is aimed at expansion and scale-up, focusing on broader dissemination and implementation, stronger and more systematic referral pathways, and systems changes that make physical activity assessment, counseling, and referral to AAEBIs a more routine part of arthritis management across health care and related professional settings.
Eligibility differs by component. Component A is limited to applicants that are not currently funded under the CDC Arthritis Program cooperative agreements CDC-RFA-DP21-2106 and CDC-RFA-DP18-1803, a restriction intended to broaden national capacity and improve geographic diversity of arthritis efforts. Component B has unrestricted eligibility. The NOFO is offered by the Department of Health and Human Services, Centers for Disease Control and Prevention (CDC), within NCCDPHP, and is categorized as discretionary funding through a cooperative agreement (CFDA 93.945).
The funding scale and expected number of awards are clearly defined. CDC anticipated making a total of about 13 awards, split across the two components. For Component A, CDC anticipated 6 to 10 awards at about $200,000 to $300,000 each (average anticipated award $250,000). For Component B, CDC anticipated 5 to 7 awards at about $375,000 to $550,000 each (average anticipated award $450,000), with an overall award ceiling of $550,000. By the end of the project period, CDC expects measurable progress in three outcome areas: increased enrollment in AAEBIs driven by sustainable dissemination strategies; increased numbers of adults with arthritis who receive provider counseling for physical activity (supported by better assessment, counseling quality, and referral practices); and reduced disparities in arthritis management and outcomes, especially among the high-burden and underserved groups named in the NOFO.
